Patient Rights & Clinical Confidentiality

HIPAA Notice of Privacy Practices

Effective Date: October 30, 2026

This Notice describes how medical and behavioral health information about you may be used and disclosed by Path To Purpose LLC (Path 2 Purpose) and how you can obtain access to this information. Please review it carefully.

1. Our Pledge Regarding Protected Health Information

We understand that mental health and substance use treatment records are deeply personal. Path 2 Purpose is required by federal and state law, including the Health Insurance Portability and Accountability Act (HIPAA) and applicable confidentiality statutes, to maintain the privacy of Protected Health Information (PHI), provide this Notice of our legal duties and privacy practices, and notify affected individuals following any breach of unsecured PHI.

2. Permitted Uses & Disclosures for Care, Payment & Operations

Subject to stricter state or federal behavioral health consent rules where applicable, we may use and disclose PHI for:

  • Clinical Treatment: Coordinating care among licensed therapists, psychiatric providers, and medical staff involved in your treatment plan, or with external providers when authorized.
  • Insurance & Payment: Verifying health plan benefits, obtaining prior authorizations, and submitting claims or superbills to your designated health insurer.
  • Healthcare Operations: Conducting clinical quality assurance, licensing compliance, accreditation review, and staff supervision necessary to operate safely and effectively.

3. Special Protections for Behavioral Health & Substance Use Records

Records relating to mental health counseling and substance use disorder treatment may receive heightened confidentiality protections under federal regulations and state behavioral health privacy laws. Outside narrow statutory exceptions such as medical emergencies, mandated safety reporting, or valid court orders, Path 2 Purpose obtains your written authorization before releasing clinical records to external parties.

4. Your Rights as a Client

You have the right to:

  • Inspect and request an electronic or paper copy of your clinical and billing records.
  • Request an amendment to health records you believe are incomplete or inaccurate.
  • Request confidential communications by alternative means or at specific contact numbers or addresses.
  • Request restrictions on certain uses or disclosures, including restricting disclosure to a health plan when you pay for a service out of pocket in full.
  • Receive an accounting of certain disclosures and revoke any written authorization in writing at any time for future disclosures.

5. Questions, Requests & Complaints

If you have questions about this Notice or wish to exercise your privacy rights, contact the Path 2 Purpose Privacy Officer at admissions@path2purposeca.com, call (209) 868.3219, or mail 2291 W March Ln Ste A115, Stockton, CA 95207.
